Sofia Vergara’s ex-husband Joe Manganiello is making shocking new allegations about their failed marriage, claiming the beloved “Modern Family” actress refused to comfort him during a devastating health crisis that nearly cost him his life.

The 49-year-old “Magic Mike” and “True Blood” star is breaking his silence about the darkest years of their relationship in his upcoming memoir, “Bloodlines,” revealing a painful struggle involving a mysterious illness, multiple surgeries, emotional isolation and the collapse of his once-glamorous Hollywood marriage.

Among his most startling accusations, Manganiello claims Vergara, 54, refused to hug him even when he pleaded for affection during his illness.

He also alleges she ridiculed a tiny dog he credits with helping him survive a period of profound emotional despair.

The revelations, published in excerpts from his memoir on October 7, have brought renewed attention to the former couple’s highly publicized 2023 split.

Manganiello is also challenging Vergara’s previous explanation that their marriage ended because he wanted children and she did not.

But behind the accusations is an even more disturbing story about a health battle that left the actor fearing he might never recover.

The former couple married in a lavish Palm Beach, Florida, ceremony in November 2015, appearing to be one of Hollywood’s happiest couples.

Vergara was enjoying enormous success on the hit ABC sitcom “Modern Family,” while Manganiello had become a household name through “True Blood” and the “Magic Mike” films.

Their relationship frequently made headlines, with the pair sharing affectionate photographs and attending glamorous red-carpet events together.

However, Manganiello now says his physical health began deteriorating shortly after their wedding.

According to the actor, he developed severe abdominal pain, persistent fatigue, fevers, headaches, rashes and other unexplained symptoms that gradually became unbearable.

At times, the pain was so intense that he struggled to walk.

His digestive system became severely inflamed, and he experienced internal bleeding as doctors searched for an explanation.

Manganiello says he underwent numerous medical examinations and procedures, including colonoscopies, while physicians attempted to determine what was happening.

He received treatment for suspected inflammatory bowel conditions, including Crohn’s disease and colitis, but the medications failed to resolve his symptoms.

Despite years of testing and medical intervention, he says doctors never established a definitive diagnosis for the underlying illness.

His condition eventually became life-threatening.

During one frightening hospitalization, Manganiello recalls experiencing extraordinary pain and fearing he would die before morning.

Doctors discovered that his intestinal tissue had deteriorated so severely that emergency surgery was necessary.

The actor underwent an operation to remove his large intestine and was left requiring an ostomy bag to manage waste.

He also lost approximately 60 pounds during his prolonged medical ordeal.

Manganiello says the experience left him physically devastated and struggling to understand how his life had changed so dramatically.

Yet he claims the deterioration of his marriage made the situation even more painful.

According to his account, the emotional closeness he once shared with Vergara gradually disappeared as his health declined.

He describes their relationship as having “become devoid of any emotionality whatsoever.”

In one particularly heartbreaking allegation, Manganiello says he desperately wanted physical reassurance from his wife but was unable to get it.

He claims Vergara refused to embrace him even when he begged her for a hug.

The actor portrays those moments as evidence of how isolated he felt while battling an illness he feared could kill him.

However, his description represents his personal account of private events. Vergara has not publicly confirmed his characterization of their relationship.

There is also an important detail in his medical story that complicates any suggestion that Vergara abandoned him throughout his health crisis.

Manganiello has acknowledged that, during a medical emergency, Vergara authorized a lifesaving surgical procedure while he was under anesthesia.

That decision allowed surgeons to perform the major operation he needed when his condition became critical.

The actor’s memoir therefore presents a complicated picture of a marriage in which serious medical decisions, emotional struggles and personal disagreements became intertwined.

But Manganiello’s revelations don’t end with his health problems.

He also claims Vergara mocked a small Chihuahua named Bubbles, a dog that he says played an unexpectedly important role in his survival.

During the darkest period of his illness, Manganiello struggled with thoughts of suicide.

He has credited his bond with Bubbles for giving him a sense of responsibility and purpose when he was having difficulty finding reasons to continue.

Caring for the tiny animal helped him focus on something outside his own suffering.

According to his account, that emotional connection became especially meaningful at a time when he felt increasingly alone.

Manganiello alleges Vergara ridiculed his attachment to the dog, deepening the emotional distance he already believed existed between them.

The claims have attracted particular attention because Bubbles had previously appeared in photographs and social media posts involving both actors during their marriage.

Still, the alleged private exchanges have not been independently established, and Vergara’s perspective on those incidents has not been publicly detailed.

The actor also reveals that his medical problems affected another deeply personal issue: whether he would become a father.

When Vergara and Manganiello announced their separation in July 2023, they issued a joint statement explaining that they had made the difficult decision to divorce after seven years of marriage.

Months later, Vergara offered her own explanation for why the relationship ended.

In a January 2024 interview, the Colombian actress said their different views about having children had contributed to their decision to separate.

Vergara, who has an adult son named Manolo from her first marriage, explained that she did not want to become a mother again later in life.

She said Manganiello was younger and wanted children, while she was ready to embrace a different stage of her life.

But Manganiello publicly disputed that explanation in July 2024, and his new memoir expands on his version of events.

According to the actor, his severe health problems had already forced him to reconsider the possibility of becoming a father.

He says he ultimately abandoned the idea of having children while he was struggling to survive his illness.

That account directly challenges the suggestion that a disagreement over starting a family was the primary reason their marriage ended.

Manganiello has maintained that the circumstances surrounding their divorce were far more complicated than the public understood.

The former couple finalized their divorce in April 2024, bringing their marriage to an official end.

Neither has publicly reconciled the differences between their accounts.

Manganiello says there were also deeply personal reasons behind his decision to finally tell his side of the story.

In an interview published October 8, the actor explained that his upcoming memoir is not intended primarily as an attack on his former wife.

Instead, he says it is an opportunity to reveal the physical and emotional suffering he kept largely hidden from the public for years.

He also wants to correct statements about his life and marriage that he believes were inaccurate.

His book reportedly explores far more than Hollywood relationships.

After conventional medical treatments repeatedly failed to bring lasting relief, Manganiello began searching for alternative approaches to his unexplained illness.

His journey included an unusual trip to Cuba, where he participated in a ritual involving a goat skull in an effort to address what he believed was happening to his immune system.

He later turned to a shaman in Santa Monica, California, who suggested that unresolved trauma passed down through generations might be connected to his physical suffering.

Manganiello credits the experience with helping him achieve a profound emotional transformation and believes it contributed to his recovery.

However, there is no established medical evidence that such rituals cure autoimmune disorders, and the precise cause of his illness remains uncertain.

The actor subsequently underwent reconstructive digestive surgery and now says he is in remission, no longer taking medication and free of the debilitating symptoms that once controlled his life.

His personal life has also changed dramatically.

Manganiello is now engaged to actress and television personality Caitlin O’Connor, 37, whom he began dating after separating from Vergara in 2023.

The couple announced their engagement in 2025 and have since relocated to the Pittsburgh area, where Manganiello has family roots.

He credits O’Connor with helping him embrace a hopeful new chapter following years of serious illness and emotional turmoil.

With his health greatly improved, Manganiello says he is even open to reconsidering fatherhood, something he once believed would never be possible.

The actor, who turns 50 in December, describes his recovery as an opportunity to begin life again after spending years expecting the worst.

Meanwhile, Vergara has continued focusing on her successful television career, including her role as a judge on “America’s Got Talent” and her acclaimed portrayal of Colombian drug trafficker Griselda Blanco in Netflix’s “Griselda.”

As of October 8, her representatives had not publicly responded to Manganiello’s latest allegations, despite requests for comment from entertainment outlets.

That means the actor’s claims about their private marriage remain his account of events rather than independently established facts.

The memoir, scheduled for release October 13, is likely to draw additional attention to what went wrong between two stars who once appeared to have a picture-perfect relationship.

But for Manganiello, the bigger story is not simply the breakdown of his marriage.

It’s the extraordinary journey from a frightening medical crisis, through years of pain and emotional despair, to a recovery he once thought was impossible.

And as the actor prepares to reveal the full story, his allegations about Vergara have already cast their former relationship in a dramatically different light.
